We’re so glad to have you returning this season. Please complete the steps below to help us start the year smoothly and safely.

  • Let us know when you are looking to participate this year!

  • Confirmation of session time will be sent by March 31st.

  • Doctor’s Form must be completed before start of session, April 28th.

    Remember, Doctor’s Forms are required BEFORE mounted activities can commence.

    Completed forms can be emailed to suzy@hilltophorizons.org‍ or mailed directly from your doctor’s office.

  • -Please submit no later than April 7th.

    -Copy of most recently completed Tax Return required

    1. Sign into Jane

    2. Go to “Intake Forms”

    3. Click “Fill Out Intake Form:

      • Participant Update

      • Financial Responsibility Form
