We’re so glad to have you returning this season. Please complete the steps below to help us start the year smoothly and safely.

Let us know when you are looking to participate this year!

Confirmation of session time will be sent by March 31st.

Doctor’s Form must be completed before start of session, April 28th.
Remember, Doctor’s Forms are required BEFORE mounted activities can commence.
Completed forms can be emailed to suzy@hilltophorizons.org or mailed directly from your doctor’s office.

-Please submit no later than April 7th.
-Copy of most recently completed Tax Return required
